Q: Who can I include on my plan?
Your can include your spouse and dependent children as defined by Florida law.
Q: How do I join the Plan?
1. Select a General Dentist. Each family member may choose the same or a different dentist.
2.
Complete your selected Plan Enrollment method (secure on-line, mail or fax).
Q: When can I use the Plan?
Plan benefits begin on the 1st of the month following receipt of your completed enrollment form. (Bank drafts on or before 15th of the prior month, annual by credit card 8 days prior to 1st.)
Q: Will I receive a Plan ID card?
Once your enrollment form has been processed, you (and all family members) are sent a Plan ID card, the Individual Dental Service Agreement, and a complete list of Copayments.Q: How do I use the Plan?
After your effective date, call your plan dental office for an appointment. Let them know you are a new "Assurant Secure Choice" member. Take your Plan ID card on your first appointment. When you go to a plan dentist, you are responsible only for the listed copayments.

You may change dentists by calling the Assurant customer service department. Change requests received prior to the 15th of the month become effective on the 1st of the following month.

Yes, you receive 25% off regular fees at participating Orthodontists (not available in all areas). Based on the cost of braces in Florida, you will typically save $1000 or more!
